ABOUT ROBOTIC SYSTEM
Operations are performed using the Da Vinci X system, one of the most advanced robotic platforms developed by Intuitive Surgical.
It was created based on three previous generations of robotic systems and is designed to ensure maximum precision, minimal invasiveness, and the highest level of patient safety.
Why the Procedure Is Considered Safe
1. Technology That Eliminates Human Error
The Da Vinci X system is equipped with Intuitive Motion™ technology and multi-axis EndoWrist® instruments that replicate the surgeon’s movements with micron-level precision, eliminating hand tremor.
This is especially critical when operating near nerve bundles responsible for potency and urinary control.
2. 35× Magnified 3D-HD Visualization
Immersive imaging enables the surgeon to clearly see the smallest vessels and tissues, significantly reducing the risk of damaging healthy structures.
3. Minimal Invasiveness
Instead of large incisions, the operation requires only 4–5 small punctures, just a few millimeters in size.
This minimizes blood loss, pain, and infection risk.
4. Intelligent Firefly® Fluorescence Imaging
The Firefly® visualization system allows real-time assessment of blood flow and tissue viability, providing an additional layer of safety during surgery.
5. Energy Control and Vascular Hemostasis
The integrated ERBE VIO dV generator and Vessel Sealer Extend instrument enable precise coagulation of vessels up to 7 mm, effectively preventing bleeding and eliminating the need for manual ligation.
Positives of Robotic Surgery
- Minimizes blood loss
- Gentle tissue handling (punctures instead of incisions)
- Short hospitalization
- Shorter surgery duration
- Preservation of urogenital function
MOST ADVANCED TECHNOLOGY APPLIED IN UROLOGY
The Robotic Surgery Centre provides a wide range of mini-invasive surgeries using the da Vinci robotic surgical system. More than 1,000 robotic operations are performed during the year using a da Vinci robot, of which 400 are prostate removal operations.
USING THE DA VINCI ROBOT WE PERFORM:
- RADICAL PROSTATECTOMY
- PARTIAL KIDNEY RESECTION
- PYELOPLASTY
- REMOVAL OF ADRENAL GLAND
- RECONSTRUCTION OF THE URINARY TRACT
- ELIMINATION OF A LARGE VOLUME OF ADENOMA IN THE PROSTATE
INFORMATION ABOUT THE POSSIBILITY OF A RADICAL TREATMENT OPTION, INCLUDING ITS ADVANTAGES AND DISADVANTAGES
Surgical treatment (radical prostatectomy):
The principle of the operation involves the removal of the prostate gland and seminal vesicles, followed by suturing the bladder to the urethra above or at the level of the muscle responsible for closing the urethra and maintaining urinary continence.
The robotic-assisted procedure has the advantage of being a minimally invasive method (performed through five small incisions in the abdomen). It has been proven that there are fewer cases of pulmonary embolism following robotic prostate surgery compared to open prostatectomy.
Adverse effects of radical prostatectomy:
A) Urinary incontinence – There is a possibility of impaired urinary control, which usually stabilizes within 3–6 months after surgery. More than 50% of patients do not use pads, 18% use pads for reassurance, and 14% use a small pad once a day.
B) Permanent loss of ejaculation – Ejaculation (release of semen during sexual arousal) is permanently lost due to the removal of the prostate, whose secretion is a main component of the ejaculate. The operation does not affect sexual sensations.
C) Potential loss of erectile function – This may occur due to temporary or permanent damage to the neurovascular bundles running on both sides of the prostate. If the tumor extends beyond the prostate and affects these nerves and blood vessels, they must be removed along with the prostate to achieve a cure. However, nerve function may be only temporarily impaired due to surgical manipulation and can recover within the first two to three years.
Various comorbidities (e.g., diabetes, vascular diseases) and the patient’s age (patients under 60 years old at the time of surgery are significantly more likely to maintain or recover erections) can negatively impact this aspect of recovery. If both neurovascular bundles are preserved, erectile function is maintained in approximately 80% of cases.
If one or both bundles must be removed, the likelihood of preserving erectile function decreases. Nevertheless, erection can be restored with medications such as Viagra or by injecting prostaglandins into the erectile tissue of the penis. This treatment is not part of the standard prostatectomy procedure.
